2012 Registration Check List:

• Complete the registration form

• Complete the photo release form (even if you DO NOT want your child's photos released)

• Payment of $30 (one session) or $50 (both sessions)

• Print out the "Appropriate Attire" list so you know how your child should dress for their camp(s)

We offer THREE options for filling out your registration forms:

1. Download Forms, Fill Them Out Electronically, Save and Email to address below

• 7 May @ Crawfish Boil & Washer Tournament
• 14 May @ Buddy's & Ace Hardware --9 am-1 pm
